Double Outlet Right Ventricle with Pulmonic Stenosis.

چکیده
The results of complete repair of double-outlet right ventricle with pulmonic stenosis in 22 Mayo Clinic patients were analyzed. The overall mortality rate was 32%. Since 1960, the mortality rate has been reduced to 16%. Anomalies of the distribution of the right coronary artery, a second ventricular septal defect of the atrioventricular canal type, and significant residual pulmonic stenosis were associated with a higher mortality rate. The difficulties in the diagnosis of this uncommon condition are emphasized. Complete repair is best carried out after the patient is 4 years old. A systemic-topulmonary arterial shunt is the procedure of choice for those who are younger than this. In the occasional patient with anomalous distribution of the right coronary artery which precludes adequate relief of the pulmonic stenosis, use of an aortic homograft from the right ventricle to the pulmonary artery should be considered.

Absence of pulmonic valve associated with double-outlet right ventricle.
ABSENCE of the pulmonic valve is a rare congenital lesion which is usually associated with tetralogy of Fallot." 2 We have recently observed a patient in whom this anomaly coexisted with "double-outlet right ventricle." This case is reported because a review of the literature has failed to reveal a similar case and because certain difficulties inherent in the differentiation of double-outlet ri...
متن کاملRight ventricular growth in a case of pulmonic stenosis with intact ventricular septum and hypoplastic right ventricle.
Adequate growth of the hypoplastic right ventricle in a patient with severe pulmonary stenosis with an intact ventricular septum was documented after pulmonary valvotomy in infancy. It is postulated that the growth of the ventricular chamber is largely the result of pulmonary regurgitation resulting from successful pulmonary valvotomy. Based on this and the observations of others on the growth ...
متن کاملThe Triply Twisted Heart: Cyanosis in an Adult With Situs Inversus, Levocardia, Double Outlet Right Ventricle, and Malposition of the Great Arteries
We present a case of a 19-year-old female presenting with cyanosis since birth. The major anomaly demonstrated was a "triply twisted heart" with a balanced physiology, allowing her to survive into adulthood. Non-invasive imaging was done using 2D and real-time 3D (or 4D) echocardiography with multi-slice imaging from 4D volume datasets. Findings were confirmed using cardiac magnetic resonance i...
متن کاملA Rare Case of Double-Chambered Right Ventricle Associated with Ventricular Septal Defect and Congenital Absence of the Pulmonary Valve
Double-chambered right ventricle (DCRV) is a rare congenital heart disorder involving 2 different right ventricle (RV) pressure compartments that is often associated with ventricular septal defect (VSD). Usually, the obstruction is caused by an anomalous muscle bundle crossing the RV from the interventricular septum to the RV free wall. We are reporting a case of double-chambered right ventricl...
متن کاملDouble outlet right ventricle with L-position of the aorta, D-loop, subaortic VSD, and pulmonary stenosis.
We describe a case of double outlet right ventricle with subaortic ventricular septal defect and pulmonary stenosis treated successfully with cardiopulmonary bypass. We consider the clinical history and angiocardiographic and surgical findings of this rare anomaly. We stress the difficulties of reconstruction of the outflow tract of the right ventricle, because of the anomalous pathway of the r...
